All details regarding the required services are provided in the Terms of Reference (ToR), which forms an integral part of this Request for Quotation.
Documents to be submitted by the Service Provider:
Duly completed and signed Supplier Registration Form (Annex A).
Annex B – Financial Proposal, duly signed and stamped (if applicable), submitted in PDF format.
Valid State Registration Certificate (Individual Entrepreneur / Sole Proprietor, Group 3 taxpayer).
Valid document confirming the applicant's tax status.
Curriculum Vitae (CV) demonstrating relevant professional experience.
Copies of certificates confirming completion of relevant training, together with documents demonstrating experience in the relevant field.
